Procedure for leave of absence, reinstatement and withdrawal from University

leave of absence | If a student is unable to study for more than two months due to illness or other special reasons, he/she may take a leave of absence upon request. The period of leave of absence shall not continue to exceed one year, and if necessary, an extension of up to one year may be granted. Please consult with your major supervisor to obtain his/her approval and obtain his/her signature on the statement of reasons. In case of injury or illness, please submit the medical certificate. |

Reinstatement to School | When the reason for absence from school, such as illness, has ceased, please submit a “Request for Reinstatement” and get a permission. Please consult with your major supervisor to obtain his/her approval and obtain his/her signature on the statement of reasons. In case of injury or illness, please submit the medical certificate. |

Withdrawal | If you must withdraw from the school due to illness or other unavoidable reasons, submit an “Application for Withdrawal” and request permission. In case of injury or illness, please submit the medical certificate. In case of reasons other than injury or illness, please consult with your major supervisor to obtain his/her approval and obtain his/her signature on the statement of reasons. |
